This Change Requests Log example is the Change Requests Log of the Quality of Life project — the example I follow in my book Project Management in 10 Steps, from the first page to the last. It is the document of Step 9 of the book. Open it to see the finished document on a real project, then write your own.
About the Quality of Life project
A personal project, run alongside a full-time job: you are the project manager and the customer. One objective — “improve my quality of life, sustainably” — with four key results (annual income, monthly balance, productivity at work, BMI), served by five deliverables: a promotion at your company, lower monthly spending, fewer unproductive hours, fewer incidents and a healthy life. The scope is known and stable, so the approach is predictive. The project runs from October 2017 to December 2018, and the documents show it as it stood on 15 December 2017.

- Requests
The two requests in this change requests log example
CR-001, 6 November 2017: add a professional certification to the promotion deliverable — choose it and get the employer's subsidy, agree a study plan with the mentor, sit the exam (new work packages 1.4 to 1.6). No impact on the project finish (the exam falls in June 2018); cost +427.50, because the employer pays half the exam fee. Approved by you, with the family's agreement, because the cost comes out of the reserve.
CR-002, 4 December 2017: add an English course to the promotion deliverable. Impact: +3 months on deliverable 1 and +1,200.00. Deferred: it does not fit the reserve together with CR-001, and it is reviewed when the exam is done.
How to use it
- Read this Change Requests Log example beside Step 9 of the book: it shows what "done" looks like.
- Open the blank template and write your own project into it.
- Or let the free Project Together course do the typing: it reads your project board and writes this same document, with your numbers in it.
